In this Privacy Notice, we describe how your Personal Data will be collected, used and shared in connection with Testing For All\u2019s project involving the distribution of certain kits to test for SARS-CoV-2 (the \u201cCOVID-19 Project\u201d). For clarity, in this Privacy Notice, \u201cSARS-CoV-2\u201d refers to the virus that causes the COVID-19 disease.<\/span><\/p>\n

Who is responsible for the handling of my Personal Data?\u00a0\u00a0<\/b><\/h3>\n

Testing For All is the Controller (for the purposes of the General Data Protection Regulation (\u201cGDPR\u201d)) of your Personal Data. Testing For All is a company limited by guarantee registered in England and Wales under company number 12570433 with its registered address at 13 Hawley Crescent, London, England, NW1 8NP.<\/span><\/p>\n

What types of Personal Data will be collected and processed in connection with the COVID-19 Project?<\/b><\/h3>\n

If you become part of the COVID-19 Project, Testing For All will collect and process Personal Data about you, which may include:<\/span><\/p>\n

    \n
  • Your full name, date of birth, sex, full home address including postcode;<\/span><\/li>\n
  • Your NHS Number;<\/span><\/li>\n
  • Email address and mobile phone number;<\/span><\/li>\n
  • Your history of symptoms associated with the virus SARS-CoV-2 and related COVID-19 disease;<\/span><\/li>\n
  • Whether you tested positive for SARS-CoV-2 and\/or COVID-19 and if so, where and when;<\/span><\/li>\n
  • Ethnicity;<\/span><\/li>\n
  • Sample (i.e. any fluid, such as blood);<\/span><\/li>\n
  • Sample ID; and<\/span><\/li>\n
  • Laboratory test result.<\/span><\/li>\n<\/ul>\n

    For arrivals from countries classified by the UK government as red, amber or green, and for travel testing such as fit to fly and test to release Testing For All may collect additional Personal data:<\/span><\/p>\n

    • Passport number and transport vessel number;<\/span><\/li>\n
    • Recent travel history including the countries visited.<\/span><\/li>\n<\/ul>\n

      If you become part of a usability study or clinical trial, Testing For All may collect and process additional Personal Data about you, which may include:<\/span><\/p>\n

      • Demographic information such as your education level and ethnicity.<\/span><\/li>\n
      • We may provide additional or supplemental privacy notices to you in respect of any such studies or trials. These supplemental privacy notices will govern how we may process the information in the context of that study or trial.<\/span><\/li>\n<\/ul>\n

        For what purposes will Testing For All process my Personal Data and on what legal basis?<\/b><\/h3>\n

        Testing For All will process your Personal Data for the following purposes:<\/span><\/p>\n

        • to distribute self-tests and\/or sampling kits to you to enable you to get tested for viral RNA and\/or antigens of SARS-CoV-2, and\/or antibodies against SARS-CoV-2 and to send you the result of your test;<\/span><\/li>\n
        • for planning effective testing strategies and testing protocols \u2013 for example, to determine the correct test that you should take (e.g. you may need to be given a different test depending on when you think you were infected with the virus);<\/span><\/li>\n
        • to comply with any legal requirements; and<\/span><\/li>\n
        • to enable the NHS and\/or Public Health England to undertake resource planning and public health monitoring and statistical analysis (including to measure the public\u2019s antibody and viral presence over time), and to inform its responses to the COVID-19 pandemic.<\/span><\/li>\n<\/ul>\n
